We would like to inform you of an important update to the Pick-Up Sheet and
associated procedures, effective immediately. Please carefully read and follow the
instructions below:
Key Changes to the Pick-Up Sheet
1. New Section Added:
● A section has been added for the Pick-Up Driver to indicate at which branch
the funeral will be arranged.
● This must always be the branch closest to where the funeral will take place.
2. Mandatory Completion:
● The Pick-Up Driver must ensure the Pick-Up Sheet is completed in full.
Funeral Process Letter for the Family
● Hand the Letter to the Family:
o The Pick-Up Driver must provide the family with the attached Funeral
Process Letter, which outlines the required documentation for their branch
visit.
o The letter is printed A5 size (two letters per A4 sheet). Ensure these are
prepared and ready for use.
New Reporting Procedure
● Photo Submission:
o After delivering the deceased to the mortuary, the Pick-Up Driver must take
a clear picture of both sides of the completed Pick-Up Sheet.
o Post the photos immediately to the Mortuary WhatsApp Group.
Action Items
1. Discard Existing Sheets:
o All previously printed Pick-Up Sheets must be discarded. Only the updated
version may be used.
2. Prepare Funeral Process Letters:
o Print the attached Funeral Process Letters in the correct format and have
them readily available for distribution.
