Procedure:
An employee is entitled to at least 4 (four) consecutive months maternity leave benefits from UIF.
Notice must be given to the employer as soon as possible or no later than 3 (three) months before the employee intends to commence maternity leave.
All employees must arrange in advance to identify a relief for the position and to arrange for takeover and training.
The employee must take maternity leave at least 4 (four) weeks before the expected date of birth, and 6 (six) weeks after birth, except if a medical practitioner stated otherwise.
An employee must notify an employer in writing of the date on which the employee intends to:
Commence maternity leave; and
Return to work after maternity leave.

An employee who has a miscarriage during the third trimester of pregnancy or bears a stillborn child is entitled to maternity leave for 6 (six) weeks after the miscarriage or stillbirth, whether or not the employee had commenced maternity leave at the time of the miscarriage or stillbirth.
The required UIF forms must be completed after receiving a medical practitioner confirmation letter.
The following are applicable and must be submitted at Department of Labour by the employee or authorised person:
Form UI-19 employers’ declaration for employees’ benefits (Available Payroll office).
Form UI- 2.2 to be completed by the medical practitioner (Available from Payroll office, please request the doctor for completion and signature).
Form UI-2.3 application for maternity benefits.
Form UI-2.7 remuneration received by the employee whilst still in employment (Available from the Payroll office).
Form UI-2.8 application to pay UIF benefits into banking account (Available from Payroll office and employee must request the bank for completion, signature and bank stamp).
Form UI-4 continuation of UIF benefits.
Maternity UIF- Power of attorney.
The employee must bring all her completed forms to the HR department before taking maternity leave and submit it before the date of birth to the UIF offices.
The employee must submit the child’s birth certificate to the UIF offices.
Please take note that the employee has only 6 (six) months from date of birth to claim UIF maternity benefits.
