First-Things-First

Congratulations! Bringing new life into the world is truly a special phenomenon and should be celebrated by all!

But, before you welcome your little bundle of joy, let's look at some important aspects to get in place first:

Report to HR

It is important that HR be notified of your pregnancy immediately in order to get things started as soon as possible as there might be several things to sort out before you go on maternity leave.

Temporary Replacement

Someone will need to temporarily replace you while you are away. Have a talk with your supervisor to come up with a plan. All options should be discussed with HR to ensure a workable plan.

Doctor Visits

It remains important to regularly have your checkups to ensure that you and your baby are healthy and doing well.

UIF Forms

The company will issue you with the applicable forms for a UIF claim. The documents below should be completed and submitted to HR at least 3 months before your due date, to assist with completing the UIF documents.

1. Application for Maternity Leave UIF Forms

Submit 3 months before your due date

2. Maternity Leave Application

Once you are more certain of your due date, complete and submit a Leave Application Form below. The dates can be adjusted should anything change.
