Implementation of Triplicate Pick-Up Sheet Book

Please be informed that the Pick-Up Sheet is now available in triplicate book format. This replaces the previous loose sheet version and has been introduced to improve tracking, control, and auditing of corpse collections.


Each form in the book is pre-numbered to enable accurate traceability when required. The process remains exactly the same as before — only the format has changed.


Distribution and Handling Instructions: • Page 1 (Original): Once the pick-up details have been posted on the Pick-Up Sheet WhatsApp Group, this page must be handed to the relevant Funeral Arrangement Officer. • Page 2 (Duplicate): To be handed by the Pick-Up Driver to the family at the time of collection.

• Page 3 (Triplicate): Remains in the book for auditing and archive purposes.


Book Distribution and Management:

• The number of books issued to each mortuary will be based on the number of pick-up vehicles available at that location at any given time.

One additional book will be provided as a backup to avoid running out of forms.

• It is recommended that each book be allocated to a specific pick-up vehicle, with the vehicle registration number clearly written on the front cover of the book.

• The extra backup book should only be allocated to the vehicle that finishes its book first.

• Mortuaries must closely monitor usage of the books and place orders in advance with the Control Room in Tzaneen to ensure continuous availability.

• When a book is completed, it must be returned to the Control Room when placing the order for a replacement.
