Implementation of Triplicate Corpse Removal Confirmation Form Books

The Corpse Removal Confirmation Form is now available in book format. This is the same form you have been using previously in loose printed format, but it has now been formalised into a triplicate, pre-numbered book for improved tracking and auditing.


Each mortuary will receive two books to begin with. Once the first book is completed, it must be sent to Abel at Head Office along with a request to issue one replacement book. This process ensures that mortuaries do not run out of forms.


Distribution and Filing Instructions:

Page 1 (Original): Must be completed and filed by the Mortuary Assistant at the time of the corpse removal. Page 2 (Duplicate): • In the case of external removals, this page must be handed to the family member. • For internal removals, it must be handed to the Driver, who is responsible for delivering it to the receiving Mortuary Assistant for filing.


• Page 3 (Triplicate): Remains in the book for record and archiving purposes.


Please ensure that all fields on the form are completed accurately and in full, as incomplete forms will not be accepted.
