Please also read through the official Company Memo to ensure you have the latest information:

All employees visiting a clinic for any reason must print out this form to be signed by the clinic staff or doctor.

If the employee is booked off, the doctor must still issue a medical certificate.

This does not constitute a medical certificate but is merely to confirm that the employee visited the clinic/doctor's rooms on the mentioned date.

Upon return to the office, the letter must be handed to the branch manager.

Please email this form back to HR and Payroll.
