Amendments on Policies – Representation by Third Parties (Affidavit Required)

INTRODUCTION
Only the Main Member may make amendments to their policy. We recognise, however, that some Main Members are elderly or living with a disability and cannot reasonably travel to a branch.
POLICY UPDATE (EFFECTIVE IMMEDIATELY)
In exceptional cases where a Main Member is unable to attend in person due to age or disability, a representative may request policy amendments on the Main Member’s behalf, provided all requirements below are met.
REQUIREMENTS (NO EXCEPTIONS)
The representative must present:
1. Original Affidavit not older than 30 days, wherein the main member expressly authorises the named representative to make the specified amendment(s) on behalf of the Main Member.
2. Copy of Main Member’s ID (clear, preferably certified within 3 months).
3. Representative’s ID (original for inspection + copy for our records).
4. Policy number and contact details for both parties.
5. Reason for representation (e.g., “housebound due to disability/advanced age”). Where available, supporting proof may be attached (e.g., medical/clinic card).
VERIFICATION STEPS (STAFF MUST DO)
● Call the Main Member (using the number on file) while the representative is present to confirm consent and the specific amendment(s). ● If the Main Member cannot speak, record the reason (e.g., speech impairment; unavailable) and ensure the Affidavit clearly authorises the representative. ● Complete the IDR form and attach all supporting documents.
WHAT COUNTS AS AN "AMENDMENT"?
Examples include: change of address/contact details, beneficiary updates, banking detail changes for debit orders, switching to Plan E, and surname/ID corrections (with supporting documentation as proof, where applicable).
IMPORTANT SAFEGUARDS
● This accommodation is only for Main Members who are elderly/disabled and unable to visit in person - not for convenience. ● No affidavit = no amendment. ● Any suspected misrepresentation must be escalated to the Compliance Department immediately. ● Handle all information in line with POPIA and internal data-protection procedures.
